In order to secure a place on the student roster the entrance fee must be sent upon receipt of the acceptance letter. Submit payment here (please add 3% charge) or through Zelle (without a charge) to admin@heritagetn.org
The following forms need to be completed and turned in before your arrival. You may choose to mail, fax or email the completed forms to the registrar.

• Complete the Parental Permission Form  (must be notarized)

• Complete the Consent for Medical Treatment Form (must be notarized)

• A physician is to fill out and sign an Immunization Record or you can fill out and sign this Immunization Exemption Form

• A physician is to complete the Medical Examination Form

• Complete the Medical History Form

• Complete the Medication Administration Form

• Read and Sign the Disaster Response Permission Form

• Read and Sign the Cell-Phone Use Agreement

• Read and Sign the Internet & Computer Agreement

• Read and Sign the Dorm Alarm System Agreement

• Read and Sign Enrollment Contract

• A copy of the student’s social security card

• A copy of the student’s birth certificate

• Take or mail the Transcript Request Form to the registrar of the applicant’s last school.

Please read the Heritage Academy Bulletin (Handbook) and Heritage Academy Standards before filling out the Parental Commitment; form is to be completed by the parent.
